Application Process

Applying for Admission to Providence St. Mel School is EASY AS 1-2-3!

STEP 1:  Application  and Benchmark Review

Parents and Students must complete all sections of the PSM Application and submit with (1) prior year grade report (2) most recent standardized test scores. PSM Assessments will be given for students applying for Pre-School, Pre-K, Kindergarten, Grades 1-3.  (3) certified copy of birth certificate

STEP 2:   Recommendation Forms & Student Summary

Two PSM Recommendation Forms are required for students applying for Grades 11th through 6th. One from prior year teacher in Reading, Lang Arts, English or Math; One from Counselor, Dean of Students or Principal.  One PSM Teacher Recommendation Form is required for all students applying for Grades 5th through 1st. The PSM Student Summary-Attendance & Discipline Form is required for all students applying for Grades 11th through 1st.

Please Note:Providence St. Mel does not accept incoming 12th grade students.

STEP 3: SMART Tuition and SMART Financial Aid Process 

Registration for the school year is the final step which includes deposit for Book, Materials, and Curriculum Resource Fee, enrollment in SMART Tuition Online Management.

Application for Admissions Requirements - Checklist

  • Providence St. Mel Admissions Application
  • A copy of prior year report card and current year progress report
  • A copy of most recent standardized test scores (Standardized scores from Iowa, Terra Nova, California Achievement, or ISAT, Explore, Plan, ACT)
  • Certified copy of Birth Certificate for all students
  • Developmental Assessment (Pre-S, Pre-K, Kindergarten)
  • Admissions Standardized Test (Applying for Grades 1st – 5th )
  • Grades 1st – 5th : One PSM Teacher Recommendation Form is required
  • Grades 6th – 11th : Two Recommendation Forms are required
  1. One from prior year Teacher
  2. One from Counselor, Dean of Students or Principal
  • Student Summary Attendance & Discipline Form (Grades 1st – 11th)
  • Student Essay Statement (Grades 5th – 11th)
  • Parent / Guardian Statement (All Applicants)
  • Family Interview (All Applicants)
  • Medical Forms (Physical, Eye, Dental)
  • Consent and Release Form
  • Official Transcript for all HS transfer students (minimum GPA 2.75)
  • SMART Tuition Setup & Family Financial Profile
